- Culture studies petition (CASE Diversity in the United States or CASE Global Civilizations and Cultures)
- Foreign language proficiency application for international students who are non-native speakers of English
- Grade replacement (Extended-X) petition
- Graduation application
- Instructional concerns form
- Intensive Writing (CASE IW) by special arrangement petition
- Intensive Writing (CASE IW) with a course taken at another IU campus or at another institution petition
- Law/medical/graduate school certifications form
- Pass/Fail application
- Release of Information
- Resumption of studies following multiple semesters of Withdrawal from All Subjects petition
- Returning student application
- Second bachelor’s degree application (for students currently pursuing a bachelor’s degree at IU Bloomington)
